Download
and print your Application for Certification
For your convenience, NCCAP offers you the ability to download
and print your Certification Application from our Web site rather
than wait for your application to arrive as a result of your phoned-in,
emailed or written application request.
In
order to download and print your Application,
you'll need to have Adobe
Acrobat Reader, a common, cross-platform utility
that comes with almost every Web browser. So
chances are, if you can read this page, you have
it and can view and print your application. If
you think you do not have it, or if you would
like to make sure you have the latest version,
click on the "Get Adobe Acrobat Reader"
button below.
To download your
Certification Application, simply click on the
"DOWNLOAD FORM" link below.
Once the
download is complete, double-click the document
"form.pdf" in your downloads folder and
allow Adobe Acrobat to open it. Then choose
FILE:PRINT from your dropdown menu, and the form
will be sent to your printer.
Once printed, fill
out the form and mail to us at the address on the
form. PLEASE make sure to fill out the form
completely and correctly to ensure processing.
