Advancing Excellence in Long-Term Care Collaborative. AELTCC

In recent years, NCCAP has strengthened its partnerships across the field, including its current involvement in member meetings with the Advancing Excellence in Long-Term Care Collaborative. AELTCC. We’re grateful for this collaborative community dedicated to strengthening post‑acute and long‑term care. AELTCC mission is to serve as a forum for government, professional, consumer, trade, clinical, labor, and other leaders to share information and education, take collective action, and develop policy recommendations on the vital issues that shape the quality of care and quality of life for individuals receiving post‑acute and long‑term care. AELTCC vision is that all individuals seeking post‑acute and long‑term care are supported in having their goals, preferences, and priorities met, and in achieving their highest possible quality of care and quality of life. They are grounded in values that guide this work every day. They act to empower and support older adults and all individuals needing post‑acute and long‑term care—along with their family caregivers. They work to combat ageism and address inequities of every kind in both access to care and quality of care across all settings, through advancing progress through shared mission and collective action. They identify and disseminate best practices and promote promising policy initiatives. They work to inform a better public understanding of PALTC and foster appreciation for those—both formal and informal caregivers—who provide care. They collaborate openly, sharing information and insights from the diverse perspectives of our members. AELTCC values and embraces diversity of views, respecting each person’s point of view as they seek consensus and common ground. During monthly meetings we engage fully, share our expertise, and continue to strengthen this vibrant community working together for meaningful change. They welcome new members.
